Nomination Form

This system-wide scholarship was established in April 2009 through the Monsignor Feeney Foundation by the classmates, friends, and family of Elisabeth Steel Reurink, a CCH student who was killed tragically on a school skiing trip on March 5th, 2009 at Blue Mountain. This scholarship is intended to be student led, student directed, and student inclusive. It is open to all Grade 12 students in all LDCSB secondary schools who are going on to post-secondary education. The winner will be required to attend the Monsignor Feeney Foundation award ceremonies in June to receive their $1,000 cheque and award.

1. Principals are requested to ask homeroom teachers of Grade 12 students to distribute the attached nomination form (p.3) by Friday, April 12th, 2013, have the students complete it, and return it to the teacher no later than first period on Thursday, April 18th, 2013.

2. Students may self nominate.

3. The Principal and School Chaplain will select one or two candidates from among the top nominations who best meet the award scholarship criteria (listed on nomination form) and notify the students of their decision.

4. The name of the candidate(s) selected, and contact information, will be forwarded by the Principal to the selection committee c/o Linda Steel via email at pdgi@sympatico.ca by Wednesday, April 24th, 2013.

5. Candidates will be interviewed by the selection committee on Saturday, May 11th, 2013 at 859 Notre Dame Drive, London, Ontario. Candidates will be contacted directly by the selection committee to establish interview times.

6. Candidates will come prepared with: a 500 word essay expressing their personal thoughts on the importance of excellence, kindness, Catholic Education and the Catholic Graduate Expectations within their lives; a copy of their 2011/2012 first term report card; two (2) letters of recommendation – one from a teacher, and one from a member of their community (i.e. coach, priest, employer, choir leader, etc.); and a copy of their resume.

7. All information will be kept confidential and all information provided by the candidates and selection committee notes will be destroyed upon completion of the 2012 scholarship process.

8. The successful candidate will be notified via telephone by 8:00 p.m., Thursday, May 16th, 2013. The school of the award winner will be notified via email to the Principal, on Friday, May 17th, 2013.

9. All other candidates will receive a letter via Canada Post recognizing their efforts and thanking them for their participation.

10. This Selection Committee will be comprised of: students of the memorial award scholarship organizing committee, a member of the Monsignor Feeney Foundation, and the Steel Reurink Family.
